Identity Medical Group is a rapidly growing Adult, Pediatric and Family Medicine physician group in Ventura County that was formed in April 2013. The vision of Identity Medical Group is driven by several guiding principles: Physician lead, Patient focus and Quality of Care. With offices in Ventura, Oxnard, Camarillo, and Santa Paula, Identity Medical Group is now the largest independent physician practice to serve this area.
Identity MSO is a full practice manager for Identity Medical Group, assisting physicians with the administration of their practice overhead, staffing, billing, revenue cycle management and IT support.
Identity Medical Group SNF Team
Identity Medical Group (IDMG) had developed a dedicated SNF Team to provide skilled and custodial management of your patients. The Team consists of a fulltime SNF Physician and Nurse Practitioners who are supported by selected Identity Medical Group Primary Care Physicians. Our Care Coordination Team Management Staff works closely with the SNF Team and Skilled Nursing Facility Staff to ensure a smooth and effective discharge plan is created for the patient and family or caregivers.
Our IDMG SNF Team is committed to providing the following:
- Initial Physician visit within 72 hours of admission
- Availability of Nurse Practitioner on-site support Monday – Friday
- 24/7 call coverage by Nurse Practitioners, Family Practice and Internal Medicine Physicians
- Assistance with discharge planning
- Appropriate referrals to Palliative Care or Hospice services
- Securing post discharge follow up appointments
- Documentation of admission, visits and discharge summary available within NextGen
- Option of home visits by Nurse Practitioners for fragile and home bound patients
- Support transitions across the continuum of care by the Care Coordination Team
